Adding File Uploads to Packages
When a parent is filling out enrollment paperwork, you may need to collect certain documents, such as a copy of the parent’s ID or their children’s vaccination records.
Additionally, when you’re hiring new staff at your center, oftentimes there are documents you may need the candidate to upload upon submission of their application.
With IntelliForms, that is possible!
When putting a package together, you have the option for parents/candidates to upload any file you need. To add this option to a package, select + Add Document and then choose -File Upload-.
Next, title the File Upload whatever it may be that you’d like the parent/candidate to upload. Choose if you’d like the File Upload to be required. Here, you will also see the option to send the File Upload as an attachment to the submission recipient upon submission of the package.

Parent Identification (passport, driver’s license, etc.)
Birth Certificates
Signed Physician Forms
First Aid and CPR Certifications
Vaccination Records
